Tzanakis Score versus Alvarado Score in the Diagnosis of Acute Appendicitis: A Comparative Study

Abstract
Acute appendicitis is most common surgical emergency. The study was conducted to compare the efficacy of Tzanakis score and Alvarado score in diagnosing acute appendicitis. The study was designed as a non randomised, prospective, observational, clinical study, conducted at Patna Medical College and Hospital, Patna, over a period of 6 months between July 2016 and December 2016. It included 98 clinically diagnosed cases of acute appendicitis who underwent emergency open or laparoscopic appendectomy. Final diagnosis of acute appendicitis was based on histopathological examination report of resected appendix. The sensitivity, specificity, positive predictive value and negative predictive value of Tzanakis score was 93.83%, 52.94%, 90.48% and 64.29% respectively and the sensitivity, specificity, positive predictive value and negative predictive value of Alvarado score was 86.42%, 47.06%, 88.61% and 42.11% respectively. The negative appendectomy rate in the study was found to be 17.35%. To conclude, clinical diagnosis of acute appendicitis can be complemented with Tzanakis scoring system for more accurate diagnosis of acute appendicitis.
