Embryonal rhabdomyosarcoma of the uterine cervix is a rare malignant mesenchymal tumor affecting mainly adolescents and young women. Clinical presentation is often nonspecific, making imaging a key element for early diagnostic suspicion. We report the case of a 26-year-old woman presenting with a bleeding cervical mass prolapsing through the vulva. Pelvic ultrasound demonstrated a solid-cystic cervical mass with a vascularized tissue component on color Doppler, while pelvic MRI revealed a large exophytic cervical lesion with heterogeneous signal intensity and marked contrast enhancement. Thoraco-abdominopelvic computed tomography showed no evidence of distant metastasis. Histopathological examination confirmed the diagnosis of embryonal rhabdomyosarcoma. This case highlights the complementary role of ultrasound, MRI, and CT in diagnosis, staging, and multidisciplinary management.
