Efficacy & Safety of a Triple Combination Chlohexidine Mouthwash Compared to Plain Chlorhexidine Mouthwash in the Management of Gingivitis: A Randomized, Controlled, Open Label Multicentre Study

Abstract
This study aimed to evaluate the clinical efficacy and safety of test mouthwash, a triple combination formulation containing chlorhexidine gluconate (0.20% w/v), sodium fluoride (0.05% w/v), and zinc chloride (0.09% w/v), in improving gingival health, plaque control, and halitosis among healthy individuals. A 28‑day randomized interventional study was conducted comparing Triple Combination Chlohexidine mouthwash with plain chlorhexidine mouthwash. Clinical parameters, including Gingival Index (GI), Plaque Index (PI), Sulcus Bleeding Index, halitosis (organoleptic) scores, periodontal pocket depth, and oral hygiene status, were assessed at baseline, Day 15, and Day 28. Safety evaluation was performed through monitoring of adverse events and tolerability. Both treatments demonstrated statistically significant improvements across all evaluated parameters (p < 0.0001). However, test mouthwash showed consistently superior clinical efficacy. The Gingival Index was reduced by 70.41% in the test group compared to 51.59% in the comparator group. Similarly, plaque scores decreased by 68.90% in the test group compared to 51.38% in the comparator group. The Sulcus Bleeding Index demonstrated a reduction of 71.96% in the test group compared to 50.48% with the comparator. Notably, halitosis reduction was more pronounced in the test group, achieving a 98.8% decrease, indicating near-complete resolution. Improvements in periodontal pocket depth and oral hygiene index scores further supported the superior performance of the test formulation. In addition, patient-reported outcomes indicated faster symptom relief and greater overall acceptability. No treatment-emergent adverse events were reported, and both formulations were well tolerated. Overall, Triple Combination Chlohexidine mouthwash provides superior oral health benefits through a synergistic combination of antimicrobial, remineralizing, anti-inflammatory, and astringent actions.
