Background: Mandibular hypoplasia following childhood trauma represents a complex reconstructive challenge. Mandibular distraction osteogenesis (MDO) has emerged as a reliable technique for skeletal and functional rehabilitation. Case Presentation: A 23-year-old male with a history of bilateral condylar fractures at age 8 developed severe micrognathia, trismus, dysphagia, and airway compromise. A combined approach including coronoidectomy and MDO was performed. Results: Significant improvement in mouth opening, swallowing improvement, airway function, and mandibular projection was observed within 3 weeks. Conclusion: Based on this experience and a search of the literatures, it is hypothesized that MDO is an effective modality for managing complex post-traumatic mandibular deficiencies with excellent functional outcomes.
