Hepatic epithelioid hemangioendothelioma (HEH) is an extremely rare tumor of vascular origin. The disease can manifest itself in several symptoms and can be diagnosed by ultrasound, CT scan and MRI. Hepatic capsular retractions due to extensive fibrosis can be identified by CT and MRI. There is no consensus on the treatment of hepatic epithelioid hemangioendothelioma, which ranges from liver resection, hepatectomy, liver transplantation. This case report presents a 45-year-old woman who presented with hepatic epithelioid hemangioendothelioma diagnosed after the onset of jaundice.

The duodenal adenomyoma is a benign lesion clinically, radiologically and during surgery simulating tumor pathology, in particular vaterian ampulloma. These similarities lead to the performance of unnecessary cephalic duodenopancreatectomy. We report a case of duodenal adenomyoma leading to cephalic duodenopancreatectomy.

Squamous cell carcinoma is a rare entity, usually presents as an indurated and / or ulcerated plaque or as a vegetative proliferation associated with a chronic inflammatory process. It is localized preferentially on the sun-exposed areas. We report an original case of invasive squamous cell carcinoma located in the right inguinal region with fatal evolution.

The avulsion fracture of the calcaneal tuberosity are rare. They correspond to a disinsertion of the calcaneal tendon and must necessarily be repaired surgically. The mechanisms of injury are forced plantar flexion or abrupt dorsiflexion. In this article, we report a rare case of avulsion fracture of the calcaneal tuberosity in a 35-year-old man. We discuss the clinical and therapeutic aspects for this lesion. Surgical treatment was realized with Percutaneous Cannulated Screw Fixation and equinus short leg cast immobilization.

Intraspinal cord lipoma is an uncommon benign tumour with an incidence of <1% of all spinal cord tumors. It is commonly associated with spinal dysraphism. However, lipoma without spinal canal abnormalities is rare with only a few case reports in the literature. Here, we report a case of intraspinal cord lipoma without spinal dysraphism in a 17-year-old male patient with progressive numbness and spastic paraplegia. Multiplanar MRI revealed a large lipoma in the dorsal spinal cord. The patient underwent surgery and was confirmed histopathologically.

Pharyngeal localizations of tuberculosis have become rare since the advent of modern anti-tuberculosis drugs and anti-bacillary vaccination, hypopharynx tuberculosis is the rarest localization of the pharynx, however it is still current in endemic countries, especially in Asia and Africa. They pose a problem of differential diagnosis with cancers. We report a rare case with primary involvement of pyriform sinus due to tuberculosis in a 54-year-old men who presented for 3 months a submandibular lymphadenopathy is reported. There were no clinical or radiological pulmonary findings. The aim of our observation is to draw the practitioner's attention to this pathology as well as the problems of differential diagnosis with hypopharyngeal cancers.

Bevacizumab has been shown to improve progression-free survival in women with ovarian cancer in multiple clinical trials. However, serious adverse effects, such deep vein thromobosis, can occur [1]. Phlegmasia cerulea dolens is a rare entity secondary to grave Deep Vein Thrombosis (DVT), in the lower extremities, and associated with rapidly progressive pain and marked swelling that may compromise the perfusion of the limb. This situation may lead to venous gangrene and the limb’s amputation secondary to a compartment syndrome [2]. We present the case of a 45-year-old female who presented eight days after the fourth administration of bevacizumab for ovarian cancer treatment, a bilateral leg pain with progressive edema which also began to exhibitwith a black discoloration. Venous dopplers showed bilateral occlusions of the common femoral, saphenofemoral, superficial femoral, popliteal, posterior tibial and peroneal veins. To our knowledge, this is the first case with bevacizumab associated Phlegmasia cerulea dolens.

Pneumothorax is an emergency therapeutic diagnostic whose bilaterality represents a severity character. The Spontaneous bilateral pneumothorax is rare but remains potentially fatal. According to Casablanca Hospital Center pneumology department, we have reported seven cases during the last six years representing 3% of total spontaneous pneumothorax patients and predominated by women gender at 31.2 years average age. Bilateral pneumothorax was primitive for one case. For the remaining six cases, it was a secondary of an asthma attack, COPD, miliary tuberculosis, cystic lung and revealed a carcinoid tumor and secondary to diffuse interstitial pneumonitis in the fibrosis stage. Facing such emergency situation, fast handling ware adopted using bilateral thoracic drain for three patients and unilateral one for three other patients. Where a bilateral pleural detachment was less than 2cm, a rest and an oxygen therapy with close monitoring was recommended. Surgery was advocated to avoid recurrency but was done for two patients. Three patients died while the other patients have progressed well.

Neurological manifestations are various and prevalent in SARS-CoV-2. Recent clinical evidences are suggests various silent neurological involvements are observed and it is an important aspect of this disease. The underlying mechanisms may includes both endothelial cell dysfunction and von-Willebrand factor inflammatory responses. Further studies must be conducted to explore the role of neurological manifestations in SARS-CoV-2 progression and must verify their underlying mechanisms. Many patients are developed a mild-to-moderate form of Neurological symptoms in the SARS-CoV-2 disease. SARS-CoV-2 mortality risk is higher in elderly patients and in patients with cerebrovascular diseases and coronary vascular diseases. Molecular mechanisms which underlie these risks are not yet understood for SARS-CoV-2. Here we discuss a possible association of SARS-CoV-2 complications with von Willebrand factor (VWF) level and endothelial damage. Von Willebrand factor is an important prognostic indicators of endothelial dysfunction and its level varies depends on sex and age. Significantly, a drug chloroquine, that showed potential efficacy for SARS-CoV-2 treatment, can influence VWF secretion and consequently its level and activities. We propose that varies in the VWF levels and its activities might be predictors of the SARS-CoV-2 morbidity and mortality, moreover the VWF might be involved in the pathogenesis of the disease. We also suggest that a comprehensive study of VWF level in SARS-CoV-2 positive groups of people with mild and severe course of the disease should be undertaken.

Motor evoked potentials (MEPs) monitoring has been applied for an early detection of the intraoperative neurologic injury. As a landmark, baseline MEPs should be accurately measured before surgical interventions. Many anesthetics, especially neuromuscular blocking agents can attenuate MEPs, and residual neuromuscular blocking effects can interfere with accurate interpretation of MEPs. Therefore, it should be antagonized completely and rapidly before measuring the baseline MEPs. We evaluated the efficacy of sugammadex in antagonizing the residual effect of rocuronium for 10 patients. In all patients with administration of sugammadex dose of 2-4 mg/kg, neuromuscular blockade was completely reversed within 5 minutes after administration of sugammadex, even in the case with deep neuromuscular block (TOF count 0). Appropriate MEPs amplitudes were measured during the surgery after reversal, with previous mild, moderate even deep neuromuscular blockade depths. The residual effect of neuromuscular blockade before MEPs monitoring in spine surgery should be completely reversed, and the sugammadex showed a good efficacy in neuromuscular blockade reversal.

Endometriosis of the wall is a very rare pathology. Its pathophysiology remains unclear. It occurs on scars from gynaecological or obstetrical surgery. We report the case of a patient presenting a cyclic pain, at the level of the caesarean section scar, at the clinical examination a mass of 7cmx5cm localized at the level of the scar of the eventration cure. Pelvic computed tomography (CT) scan established the presence of a hypodense formation, measuring 75/55 mm extended over 40 mm, which rises in its periphery after injection of contrast agent, opposite the operative scar of the eventration cure at the expense of the skin tissue. The treatment was surgical with complete removal of the lesion and a pathological study was performed which confirmed parietal endometriosis. The postoperative follow-up was simple. Through our case, we draw the practitioners’ attention to the great interest of evoking the diagnosis of parietal endometriosis each time a patient presents a mass of the wall with cyclic pain, in the close or distant aftermath of gynaecological surgery. This should systematically lead to a wide excision, followed by parietal reconstruction using prosthetic reinforcement if necessary, because of the risk of cancerization, the prognosis for which is considered to be poor.

Drug-induced acute pancreatitis is a rare pathology with a pathophysiology that is poorly understood. The diagnosis is made after eliminating the other classic causes of pancreatitis. The treatment behind the acute pancreatitis should be stopped permanently. We report a new observation of a case with acute drug-induced pancreatitis after a prolonged use of tamoxifen.

Herpes simplex virus (HSV) has various presentations, depending on the patient’s immune status, age and the route of transmission. In adults, HSV-1 is found generally in the oral area and HSV-2 is commonly found in genital area. HSV-2 without genital lesion is uncommon. Patients with immunodeficiency are at an increased risk of developing severe herpes simplex virus (HSV) infection and atypical presentation. Herein we report a unique case of cutaneous herpetic infection during postpartum period of a diabetic women caused by HSV-2 without genital involvement.

Hepatic perfusion disorders are focal or diffuse abnormalities of the parenchymal enhancement after injection of contrast agent. Superior vena cava thrombosis disease is relatively rare due in most cases to external compressions of the superior vena cava (SVC) by a mediastinal neoplasm: lymphadenopathy, mediastinal tumor. Several venous collateral pathways usually can develop allowing blood to return to the right atrium such us systemic-portal-shunting which are rarely involved. This leads to an increase flow sat the lower vein cava and the liver. We will expose in this work through two cases of thrombosis of the superior vena cava, the development mechanisms of the various collateralities involved creating systemo-portal anastomoses at the origin of these perfusion disorders as well as their translation in computed tomography.

The placement of an epidural catheter has become increasingly used in anesthetic practice. However, this procedure is not exempt from complications. Epidural catheter rupture and retention, although uncommon and rare complications, remains an area of utmost di¬lemma to practitioners. Herein, we describe the occurrence of such an event where an epidural catheter was blocked from its epidural insertion and complicated by a rupture.

Corona virus infection is an acute respiratory condition caused by a new human corona virus (SARS-CoV-2, the virus responsible for COVID-19. This virus belongs to group 2b corona viruses. Transmission of this disease appears to occur through droplets, dispersed in the environment, coughing or sneezing. Human-to-human contamination occurs through manual contact or contact with secretions deposited on equipment and other objects of common use such as doorknobs of offices, transport cars. The virus spreads rapidly, leading to an escalation in the number of cases in the population. The application of barrier measures is a response to contain the emergence of new cases. This study presents a statistical analysis of the epidemiological profile of new cases, mortality and the number of patients cured in Cameroon. It emerges that the pandemic linked to the new corona virus is in a phase of exponential contamination in Cameroon. This situation calls on health sector actors and the government to adopt a rigorous policy to halt the evolution of the disease in the country.

Chilblain Lupus Erythematosus is a rare form of cutaneous lupus erythematosus (CLE) more frequently seen in middle aged females. It is characterized by erythematous to violaceous plaques over the acral areas induced by exposure to cold, unlike photosensitivity cutaneous CLE lesion. The authors present a case of chilblain lupus erythematosus in a middle age female, it’s clinical diagnosis and therapeutic options. The differential diagnoses of a purpuric skin rash include chilblain lupus erythematosus, idiopathic chilblains and lupus pernio.

Background: The infectious epidural abscess by Streptococcus agalactiae is a rare, and a surgical emergency with a reserved prognosis. We report, to our knowledge, a rare case of S. agalactiae epiduritis. Case report: This observation reports the case of a diabetic patient who presented with compressive neurological signs. The biological assessment showed an inflammatory syndrome, and a magnetic resonance imaging objectified epiduritis. The bacteriological study of curettage biopsy made it possible to isolate a S. agalactiae. The evolution was favorable under antibiotic treatment with disappearance of neurological and infectious signs. Conclusion: Epidural abscess with S. agalactiae must be evoked with a cytobacteriological study in front of a suggestive symptomatology, especially in case of favoring factor.
