Acute Peritoneal Dialysis is Life Saving in Critically ill Acute Kidney Injury Patients: Single Centre Experience

Background: Use of Acute peritoneal dialysis (PD) is underutilized in treatment of critically ill acute kidney injury (AKI) patients admitted at intensive care units (ICU). At places where bedside Hemodialysis is not available in ICU, acute PD may provide a useful and equally effective alternative renal replacement modality. Methods: Acute PD sessions performed at our ICU during last 3 years, were analyzed. Demographic profile of patients, various complications reported, and patient outcome was studied. Results: Total 432 Acute PD sessions were performed during study period. Mean Duration was 43.8± 14.5 hours. Poor outflow was the most common complication observed (n-33, 7.6%), while peritonitis was observed in 26 (6.01%) cases. Mortality rate was 38.7%, but none of the death was directly attributed to acute PD complications. Mean Urea reduction ratio were 51.3% and 62.9%, at 24 hours and at end of sessions, respectively. Conclusion: Acute PD is an efficient and cost-effective renal replacement modality in critically ill ICU patients with minimal complication rates. In resource poor settings, it provides a viable alternative to hemodialysis.

Introduction: The caesarean section rate has increased to an alarming extent in the last three decades all over the world. In USA it was 5% in 1970 to 31% currently. Vaginal birth after previous caesarean delivery represents one of the most significant changes in obstetric practice. Objective: The objective of this study is to determine the frequency and mode of delivery in women with one caesarean section and to evaluate the feto-maternal outcome of trial of labour of previous one caesarean section. Methodology and Materials: This was a prospective study which was carried out in the department of Obstetrics and Gynecology of Sher-E-Bangla medical college and Hospital, Barisal from 1stAuguts 2007 to 31st July 2008. A total of 116 women with a history of caesarean section were delivered in the maternity unit II. Among them 50 cases were selected for trial of labour. Selection criteria were subjects with uneventful pregnancy, adequate maternal pelvic dimensions, vertex presentation and spontaneous onset of labour with previous one uncomplicated LSCS. Patients with classical caesarean section, medical complications, multiple pregnancy, IUGR placenta praevia were excluded from the study. Informed consent was taken from all patients. Maternal and fetal monitoring as carried out with facility of operation theatre, anesthesia and pediatrician. Results: Successful vaginal delivery was achieved in 44% assisted or UN assisted and repeat emergency caesarean section was carried out in 56% of the patient. Leading indication for repeat caesarean section was failure to progress, fetal distress and scar tenderness. No maternal death and one perinatal death occur. Conclusion: We conclude form this study that antenatal booking and follow up, careful case selection for trial of labour will achieve successful maternal and perinatal out come and this is an important component of efforts to lower the rate of repeat caesarean birth.

Objective: The Primary Objective of the study was to establish the relationship between Mode of Delivery and subsequent Intrapartum outcome [Fetal Distress]. The findings of the study showed higher rate of caesarean section during labor in fetuses with <20th centile of UV flow rate. The women who had spontaneous vaginal delivery had flow rates above 80th centiles and had less fetal compromise. Study Design: A prospective observational study done at SMS MEDICAL COLLEGE, jaipur. Fetal biometry and Arterial Doppler and venous flow rates were measured in 90 women immediately before established labor. Labor was then managed according to local protocols and guidelines, and intrapartum and neonatal outcome details were recorded. Results: Fetuses delivered by cesarean section for fetal compromise had significantly lower umbilical venous flow rates than those born by spontaneous vaginal delivery. Fetuses with the lowest umbilical flow rates were more likely to require emergency cesarean for presumed fetal compromise than those with the highest flow rates. Conclusion: Asssessment of umbilical venous flow can predict the diagnosis of intrapartum fetal compromise and need for emergency delivery.

Background: De Quervain tenosynovitis “mommy’s thumb” is a common disease that affects women more than men. The pain can result in decreased level of daily activity. Conservative treatment consists of physiotherapy, occupational therapy; use of orthotics, corticosteroid injection, with no one is superior to others. Objective: The aim of this study is to compare the outcome of two forms of conservative treatments for De Quervain tenosynovitis as a first line treatment. Design: A retrospective study was conducted at Princess Rashid Hospital in IRBID/ JORDAN. Methods: The files of 131 patients who has visited the PMR clinic and diagnosed to have De Quervain’s tenosynovitis in prince rashed hospital between 5/2018 and 9/2019 were reviewed. According to the files data, 74 patients’ files were eligible for the study according to inclusion criteria. The 74 files were divided into two groups, the group A included the patients who were treated with physiotherapy (active pain-free range of motion exercises, strengthening, tendon gliding, self administered friction massage and eccentric training exercises) in addition to thumb spica. The group B included the patients who were treated with one steroid injection (1 cc Betamethasone, 1 cc 1% lidocaine). The follow up was done after two months of treatment initiation. And the outcome was compared in the two groups. Result: 74 files were reviewed in this study. The mean age was 53, 33 ± 14, 01 (26-80). Of the 74 patients the males were 27 patients (36, 48%) with mean age of (55, 29 ± 15, 02) and the females were 47 patients (63, 51%) with mean age (52, 21 ± 13, 26). The group (A) (physiotherapy and thumb spica) was 45 patients (60, 81%). and the group (B) (steroid injection) was 29 patient (39, 18%). The patients in group A showed a non significant improvement in VAS pain score and in pressure pain threshold scale at two months evaluation after the treatment .from 7,37 ± 1,35 to 7.00 ± 1.38(p>0.05)and from 3,08 ± 0,75 to 2,75 ± 0,

Background: Obesity has affected around 135 million people of India. Overweight is a precursor of obesity. Prevalence of obesity and overweight varies due to age, gender geographical environment and socio-economic status of the subjects. Understanding the prevalence of subjects with overweight and obesity will give an idea of its intervention. Aims and Objectives: To study the prevalence of overweight and obesity and find out the parameters associated with the obesity. Material and Methods: Two hundred subject/patients were studied at Out Patients Department subjects/patients of Department of Medicine and Department of Pharmacology, L N Medical College and research Center Bhopal, Madhya Pradesh from March 2017 to February 2018 between. Subjects/patients age, sex, habits, socio-economic status, marital status and details on life style were recorded. Results: In present study, we found the prevalence of overweight to be 37.8% and that of obese was 17.8%. Overweight/obesity was more prevalent among female (55.55%). Factors associated with increased prevalence were inactive lifestyle (66.67%), alcohol consumption (58.33%), married life (42.5%) and old age. Conclusions: Prevalence of overweight and obesity in our center is high. Obesity was more common in female. Factors associated with increased prevalence were inactive lifestyle, alcohol consumption, married life and old age.

Purpose: This study was done to find out relationship of dengue infection with climatic factors &clinical status of the patients who were serologically positive for dengue and their clinical outcome during the year 2018-2019 from March2018 to October2019. Dengue is endemic all over India. Confirmation of Dengue viral infection is the most important & essential prerequisite for managing complications associated with dengue viral infection. Infection with the DENV serotypes may be asymptomatic in majority of cases or may result in wide spectrum of clinical symptoms ranging from mild flu like syndrome to complications such as coagulopathy, increased vascular permeability & fragility. Materials and Methods: Serum samples were collected from 1600 patients presenting, with febrile illness clinically consistent with dengue infection. Serological confirmation of dengue infection was done using IgM ELISA dengue NIV pune kit &NS1 Ag detection by ELISA. Results: Out of 1600samples, 356 were confirmed serologically positive, of which 219 were IgM positive and 137 were NS1 antigen positive. Large proportions were adult Males. Outbreak coincided with post monsoon period. Among 300 positive patients all presented with fever but duration of less than 5 days were 137 (45.6%) rest had fever more than 5 days , fever with rash were 152(50.6% ) & fever with thrombocytopenia were257(85.6%) &on discharge platelet count reverted to normal 263(87.6%) cases and some cases were referred to higher center for further management of DHS&DSS. Cocnclusion: This study highlighted climatic factors as major criteria responsible for viral infection & some important information in clinical & laboratory data that can be used for early prediction of on coming dengue fevers, early diagnosis & subsequently for appropriate treatment of dengue fevers and preventive measures.

Background: Despite being extremely common medical problem in India, the pathogenic mechanism of snake-bite induced acute kidney injury (AKI) remains elusive. Analysis of the risk factors for snake-bite induced AKI may be helpful in AKI prevention and early treatment. Methods: The study included 201 snake-bite patients who were hospitalized to our hospital during period of January 2019 to March 2020. The patients were divided into AKI and non-AKI groups. The Chi-square test, Student’s t-test were used for statistical analysis. Results: The mean age of patients was 40.5±9.5 years. Of the 201 patients 40.8% cases, (n=82) suffered from AKI. While, 33.3% (n=67) case suffered from severe AKI and needed renal replacement therapy. There were statistically significant differences between the AKI and non-AKI groups with respect to older age, female gender, time interval from snake bite to antivenom therapy, creatine kinase, Lactate Dehydrogenase (LDH), hemoglobin level, and presence of diabetes. Occurrence of AKI was very significantly associated with mortality risk (P value 0.0001). Conclusion: Occurrence of AKI was linked to advanced age, female sex, markers of hemolysis and muscle injury, delay in anti-snake venom therapy, and diabetes.

Blastic plasmacytoid dendritic cell neoplasm previously known as CD4+/CD56+ blastic NK cell tumors or hematodermic tumors. We report a case of a A76-year-old women presented with disseminated violaceus nodular, ‘bruise like‘ patches and macules of the trunk and subconjunctival hemorrhage. Immunohistochemistry showed a CD4+/CD56+ expression which confirmed the diagnosis of BPDCN was made. We emphasize that careful analyses should be performed to make the correct diagnosis.

Introduction: Role of matching of HLA- antigen in the outcome of Kidney transplantation is a well-established fact. But properly matched donor selection is a difficult job. In order to observe the role of HLA-antigen matching in renal transplantation, this study was done in Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. Materials and methods: One hundred live related Kidney transplant cases were included in this study. All cases were divided into two groups, Group 1 (n=74) having much better HLA matched than Group 2(n=26) (p<0.001). The cases were ovserved retrospectively for one year from the data of transplantation. Results: After one year, level of serum creatinine was significantly higher in Group 2 in comparison to Group 1 (147.29 ± 33 µmol/lit vs 273.07±50 µmol/lit, p < 0.05). Rate of Acute rejection was higher in group 2, than Group 1(22.97% vs 57.70%, p < 0.001). Graft survival was significantly better in Group 1 than in Group 2 (90.54% bs 65.88%, p < 0.05). Conclusion: Kidney transplantation should be done with good HLA matching between donor and recipient. As poor HLA matching is found to have poor outcome and better HLA matching in live kidney transplantation is found to have better graft outcome.

Although adenoid cystic carcinoma (ACC) is the most common tumor of the minor salivary glands, it’s relatively rare to affect the lower lip. Its non-specific clinical presentation may pose a diagnostic dilemma. Therefore, biopsy and histological examination are of extreme importance to establish the final diagnosis. In this paper, we report a case of ACC of the lower lip occurring in 35-year old patient mimicking a benign lesion.

Ultrasonography (US) of the eye is a useful tool in diagnosing conditions of the ocular globe it is characterized by availability, ease, and cheapness. This study aims to study the diabetic eye disease using ocular B-mode ultrasonography. A total of 300 (141 females and 159 males) Sudanese patients with long-duration diabetes mellitus were investigated in the period spanning from 2016 to 2019. Nidek (Echoscan US – 4000) ultrasonic unit with a high-frequency transducer 10 MHz was used to examine patients with clinically suspected eye abnormalities at Makkah Eye Complex, Khartoum, Sudan. A Chi-square test was used to assess the relationship between qualitative variables. In 91% of patients with type II the prevalence of significant types of sonographic findings in diabetic eye disease was as follows, males were more affected p-value (0.01). A Correlation is done between duration of disease and final diagnose, it was found that there was no significant relationship between them p-value (0.452). There was no significant correlation between type 2 and ultrasound findings p-value (0.204), there was a relationship between patient history and final diagnose p-value (0.000), Comparing between controlled and non-controlled DM, there was no significant correlation p-value (0.224), and (77.3%) of patients are treated with insulin. The study concluded that the ocular B-mode ultrasonography is an effective method of diagnosing diabetic eye diseases by detecting a wide range of diabetic eye disorders.

Objective: In this study our main goal is to evaluate the importance of biopsy and FNAC as a diagnostic tool in cervical lymphadenopathy cases. Method: This cross-sectional analytical study was done Total 260 patients with cervical lymphadenopathy, persisting for >2 weeks either localized or generalized attending inpatient and outpatient department (particularly surgical, medical, pediatrics, ENT, dermatology) of Dhaka Medical College Hospital, from June 2014 to July 2015. Results: The peak age incidence of cervical lymphadenopathy was observed between 2nd and 3rd decade of life (25.4% in 10 – 20 years and 25% in 20 – 30 years). Another peak was found in 50 years or above. The median age was 28 years and the lowest and highest ages were 5 and 66 years respectively.54% of patients were diagnosed clinically as having tubercular lymphadenitis, 27.7% metastatic carcinoma, 12.7% lymphoma and 5.0% non-specific reactive hyperplasia (NSRH). Conclusion: From our result, we can conclude that, diagnosis of lymphadenopathy is a team effort of the physician surgeon and the pathologist. If there is no regression in size or size increases, open biopsy can used for diagnosis in cervical lymphadenopathy cases.

The aim of this study to Estimate the patient’s dose from 18F-FDG (18F-fluorodeoxyglucose positron emission tomography/computed tomography) whole body investigations. The dose calculated using RADAR Medical Procedure Radiation Dose Calculator to estimate the effective dose, for 156 patients (110 males and 40 female) were examined by Discovery PET/CT 710, GE Medical Systems in Kuwait Cancer Control Center. The results showed that variation in effective dose, were the effective dose ranged from 156 to 9.94 mSv. And found that the effective dose for female 3.88 mSv was higher than the dose for male 3.71 mSv, this variation come from the higher value of BMI between the females 28.49 kg/m2 than the BMI of males 26.50 kg/m2, also there was lightly variation of effective dose between the right and left lung, were the effective dose for right lung 3.86 mSv was higher as same as the BMI 27.19 kg/m2 was higher than the dose 3.59 mSv and BMI 26.82 kg/m2 of left lung. The results provide that there is no difference demonstrates in the effective dose from 18F-FDG in male and female patients. And recommended that all the clinical practice should be justify and be careful about the concept risk-benefit ratio to any and efforts 18FDG whole-body PET/CT scan.

Background: Stroke in young poses a major health problem. Stroke in young has special significance in developing countries. Aims and Objective: To study the prevalence of stroke in young and compare with the normal population. Materials and Methods: Ninety subjects were studied in the Department of General Medicine at Sri Aurobindo Medical College and PGI, Indore (M. P.) for one and half year from June-2015 to March-2016 after dividing them in to Cases (n=45; patient who had ischemic stroke screened through CT/MRI findings) and Control (n=45; normal individuals who had no documented heart disease). Clinical information including age and sex were obtained and all the subjects were divided based on different age groups and the prevalence was recorded. Results: Out of a population of ninety, more than half (56, 62.2%) of the subject was male while rest (34, 37.8%) was female. The age of all cases and controls found to be in the ranges from 15 to 45 years. The mean age (mean ± SD) of all samples (N=90) was 36.53±7.53 years. The scatter of mean age for the case group (n1=45) was 36.80±7.90 years and found within ranges from 15 to 45 years while for controls (n2=45) was 36.27±7.22 years had ranges from 20 to 45 years. Conclusion: Stroke in young in present study is high. Preventive measures could aid immensely in bringing down costs and emotional burden on the family.

Objective: In this study our main goal is to assess treatment outcome of patients with large gut obstruction. Method: This prospective study was done intotal 50 patients possessing the symptoms and signs of suspected large bowel obstruction irrespective of age and gender included in this study at different surgical units of Shaheed Ziaur Rahman medical college hospital, Bogra from May 2009 to April 2010. Results: Age of the patients ranged from 14-79 years. Among them maximum (28%) were in between 51-60 years and followed by 41-50 years (24%). It was shown that large gut obstruction is more common in older age group. Volvulus (46%) is the most common causes of large gut obstruction followed by malignancy (36%). Wound infection is most common complication, followed by stoma related and chest infection.17 patient (37.77%) left hospital within 7-10 days, 12 patients within 11-15 days and only 5 patients stayed more than 21 days in whom secondary suture were done. The mortality rate was 5 patients (10%) among 50 patients. Conclusion: A careful approach is required to avoid the increased morbidity and mortality associated with delay in the diagnosis of gangrenous obstruction. The single most antecedent of a grave prognosis is late presentation. Increasing the patient awareness will help to decrease the incidence of large gut obstruction significantly?

The tenosynovite of Dequevain remains among the principal causes of the pains of the radial edge of the wrist. To relieve the patient of this pain source of discomfort and functional impotence was since the era of Dequervain the concern of the experts. If the majority of them plead until our days for medical assumption of responsibility, others nevertheless direct their attitude towards an early surgery like radical treatment without repetition. Elsewhere, much from authors concluded with the relation between the failure from the medical treatment and the existence from anatomical variations, particularly the existence of a septum intratunellaire insulating tendon EPB in a tight compartment. It is to define the place of the surgery in therapeutic, that we propose an analytical study of eight cases of TSDQ treated surgically in the service between 2014 and 2019. Among our patients, we carried out successfully the technique of fixing of the scrap capsulaire to the derm after the realization of a synovectomy and/or the résection of a septum or a musculo-tendineuse anomaly. In the light of our results and those of the literature, it appears clear that the surgery offers an effective solution in the therapeutic management of the TSDQ and deserves to be regarded as the treatment of 1st intention in the cases seen tardily.

The chondroma of the hand is a benign cartilaginous tumor that results from an absence of normal enchondral ossification below the growth plate. Most often it is unique, but also realizes multiple forms or enchondromatosis. The risk of sarcomatous transformation justifies surgery. The authors report a retrospective study of 11 cases of hand chondroma collected at the Traumatology Orthopedics II service of the Mohamed V Military Hospital in Rabat over a period of 6 years, between January 2014 and December 2019. Our present work aims to study the epidemiological, clinical aspects, iconographic, histopathologic features and pronostic of hand chondroma, to report the experience of service in the surgical management of these tumors and to discuss our results by comparing with data from recent literature.

We report a series of 32 cases of the recurrent shoulder dislocation for whom the Latarjet surgical procedure which marks our experience in the department of Orthopedic and Traumatologic surgery of Mohammed V military hospital in Rabat from 2016 to 2019. The objective of this retrospective work is to provide an update on the surgical treatment of recurrent shoulder dislocations by Latarjet procedure, to appreciate the functional result, to seek the specific complications and to target of it the principal indications while comparing our results with the data of the recent literature.

Background: Patients with chronic obstructive pulmonary disease (COPD) have raised serum levels of C- reactive protein (CRP). This may be related directly to COPD and its associated systemic inflammation or secondary to other factors such as concomitant smoking status. The aim of this study was to evaluate infection and inflammation are potential causes of raised CRP levels in COPD and also evaluate relationship between increased counts of inflammatory cells in circulation and serum CRP levels. Results: Patients with COPD acute exacerbation were characterized by a systemic inflammatory process indicated by an increased leucocyte count (15.2 ± 6.4) vs (8.6 ± 3.1) × 103 cells/µl), raised levels of CRP (32.1 ± 26.0) vs (3.42 ± 2.70) µg/ml). After treatment of disease exacerbations, systemic levels of CRP (on the day of discharge) was significantly reduced (5.42 ± 3.20) compared with day of admission and with the control group (3.42 ± 2.70). Conclusion: CRP levels are raised in COPD patients with acute exacerbation and also in stable patients at the time of discharge without clinically relevant any other chronic inflammatory diseases. CRP may be a systemic marker of the inflammatory process that occurs in patients with COPD.

People with diabetes are at increased risk of cardiovascular, peripheral vascular and cerebrovascular disease. A number of studies have investigated the effect of diabetes mellitus on arterial stiffness but very limited data on arterial stiffness is available in the Western Rajasthan and thus, we aim to investigate the type 2 diabetic related progression of arterial stiffness. This case-control study was carried out on 240 subjects were divided into two groups i.e. control 120 and diabetic 120 (60 male: 60 female) (25-60 years). To assess the arterial stiffness by Pulse Wave Analysis (PWA) and Aortic Pulse Wave Velocity (PWV). Our study concluded that the presence of diabetes is associated with increased arterial stiffness and it may contribute in part to increased cardiovascular risk in diabetic patients, when compare with control and statistically highly significant (p<0.001).

Previously considered pathognomonic for subacute bacterial endocarditis, white center retinal hemorrhages or Roth spots are currently recognized in connection with several pathologies. We report a case of a patient with white center hemorrhages due to medullar aplasia. Roth spots constitute a retinal morphological manifestation that can occur during several pathological conditions, some of which can be life-threatening. Hence the interest of a serious etiological investigation in the presence of these tasks in the fundus examination. The role of the ophthalmologist is important at the time of diagnosis and during post-treatment surveillance.

To our best knowledge, no rigorously designed clinical trials have evaluated the relation between nutrition and the large family of Coronaviruses. Drawing on experience from prior zoonotic Coronaviruses outbreaks, public health specialists have initiated preparedness and response activities to define and collect all the required knowledge about this pandemic. In this review, the authors try to introduce the most reliable scientific evidence that connects the nutrition and COVID -19. This review will summaries the nutritional management in ICU patients infected with COVID-19, vitamin supplements and COVID-19, and pre-exciting diseases and COVID-19.

Dietitians play an important role in the reduce of ,morbidity and mortality rates by effective nutrition support especially in hospitals whether in the words or in intensive care unit to detect and to prevent or curtail the metabolic complications of overfeeding by identifying patients at risk. The multidisciplinary team inside ICU It brings together nutritional experts from medicine/surgery, nursing, dietetics and pharmacy that can co-ordinate the contributions of each of these specialties into coherent nutritional support. The aim of this study was to confirm the dietitian role in parenteral nutrition administration among a multidisciplinary team according to international organizations associated with parenteral and enteral nutrition. An extensive literature review was conducted across internet using the Google scholar, books as well as a various literature databases written in English. The search was conducted based on a diversity of key words related to the review topic. The dietitian evolved from the nursing profession and established herself as an authority on nutrition and dietetics.in conclusion all associations of parenteral and enteral nutrition have repeatedly recommended the development of multidisciplinary nutrition support teams, which include dietitians, to optimize management of patients on parenteral nutrition and to improve patient outcomes and save costs.

In this review, the unique peculiarities of the SARS-CoV-2 were screened, in an attempt at defining the most relevant features of the virus that might explain its erratic and deviant behavior. The characteristics of COVID-19 were likewise scrutinized, in order to associate them with one of the footsteps of the virus in its voyage around the planet. Although a hypercytokinemia is certainly an inherent part of the disease, we have suggested that the 'cytokine storm' may not be an integral part of COVID-19, and, while microthrombosis may be accounted for by the CAC/DIC secondary to viral sepsis, various degrees of bleeding have been reported.

Introduction: Under the NHM, both supply side and demand side interventions are implemented to improve accessibility and utilization of RCH services by the urban and rural population. These interventions have contributed in improving utilization of RCH services. However, utilization of RCH services and adverse health outcomes among slum population remains a cause of concerns. Objectives: This study aims to find out the role of institutional and community level factors in deciding accessibility and utilization of RCH services. Result: The study finding reveals that 90.5% got ANC registration in which 82.9% in government hospital and 7.6% in private hospital. Among study population 33% got ANC registration in first trimester and 48% in second trimester, 90.3% mother got two T.T injections, 68.6% of mothers got regular iron and folic acid tablets and 1.2 % mothers done four or more USG. The study population had institutional delivery of 83%. Conclusions and Recommendation: The study finding reveals a statistically significant association between distances of health facility and number of ANC. There is statistically significant association between mother education, spouse education and distance of health facilities with number of USG done. Distance of health facility and place of delivery are also statistically associated. So, specific intervention program need to be planned and conducted to improve the maternal health practices and eventually improve the health status.
